The Case Worker builds on a good relationship with Health facilities and caregivers to identify/monitor children and adolescents living with HIV (CLHIV) currently receiving ART, HIV-positive Pregnant and Breastfeeding Women (PBFW), and HIV Exposed Infants (HEI) in HIV treatment facilities to achieve the following: Provide services (based on the family-centered case management) to project participants who were carried over from quarter four (Q4) of the previous year; Enroll into the project new CLHIV, HIV-positive PBFW, and HEI who are resident in the newly assigned health district; Enroll the caregiver(s) and other children <18 years as well who reside in the same household as the CLHIV, HIV-positive PBFW, HEI; Conduct needs assessment of all those enrolled and HIV risks assessment for those whose HIV status is unknow.
